At Pembroke Health + Wellbeing we now offer this convenient solution for those of you who are housebound or simply don’t have the time to come into the clinic.
How to get started
Step 1
Email or phone us to set up a convenient appointment time
Step 2
Once an appointment has been set up we will email you a health questionnaire to complete and send back prior to your consultation. This questionnaire is designed to give an overall picture of your health as well as allowing you the opportunity to define your health needs/goals
Step 3
At your appointed time we will call you for your nutrition consultation which will involve a detailed analysis of your current health and dietary habits (as in a normal clinic situation). The consultation will last approximately 1 hour.
Step 4
Based on your consultation we will email you – within 2 days – a detailed dietary and lifestyle programme to follow.
Step 5
A follow-up consultation will be organised for 2 weeks later to check how you are getting on and make any necessary changes. This consultation will last approximately 1/2 hour.
